Enrollment Requirements

  • $50-$65/ Family Enrollment Fee

  • Full Completion of Christios Enrollment Packet 

​

  • A copy of the child’s Birth Certificate

​

  • A copy of the child’s most recent shot record

​

  • Child’s Health Appraisal (15-day completion after enrollment)

​

  • A copy of Parent ID/SSN
